It’s not black or white. You need to give more info for a meaningful advice.

depends on the range of your/your enemies health to be honest… (and your damage output without it)

like, if you deal about 1-5 damage per attack (and your enemies have more than 10 health point on average), obviusly keep it…

if you deal damage in the fifties per attack, it is obviously a needless item.

if you have 20 or less health, I would argue against using it. (Unless you have a very high parry chance.)

if you have 4-digit health, you probably don’t lose much.

(I left out the middle ground options, cause they are just too much to consider in those scenarios.)

The two main question: How many turns (opporunities, etc) you gain, if you win (if your attacks are succesful) ? How many turn you lose (if you take damage)?

If the answer for the first question is way higher than the second, it is usually an useful item. If the second answer is way higher, it is probably a hindarance.

if the answer is in the middleground, the main factor is usually preference on playstyle.

okay so thats all cool i kept it and won

(my hp was 4+3, it was objectively the right option when i got another +1 buff, since heaven or hell basically means I have 3 hp whilst everyone else has 1)

it’s a non-turn based game, similar to a street fighter
it’s a roguelike, so if you die you die

The formula is the same for not turn-based games as well. Just insert attacks, instead of turns.
(It’s a bit skewed, since it depends also on your skill, how frequent the bonus damage on you or the bonus damage on the enemy applies.)
